Don: If you get a USB unit, typically it will come with a USB docking cradle (with a USB cable). The ability to get the radio/modem out away from the computer "up and into the clear" will probably be enough to make it usable. For example, that's what I did when I was on a boat - connected directly to my laptop, I wasn't getting enough signal. I put it in the docking cradle, set that on top of a seatback (where it could receive signal from a window) and it worked.
Also, if the "broadband" signal doesn't work, often the "narrowband" signal will - which will be slower (GPRS on GSM systems, 1xRTT on CDMA systems). If you're deep enough within a building... no signal is no signal, and an external amp and antenna probably won't help. I simply haven't bothered with external antennas / amps - to date, I haven't needed them, and that's yet one MORE set of things to lug around. Not to mention that the external antenna connectors are tiny and fragile... thus a pain.
