I would not say it's hardware, especially if you get a link to the switch.

On slide 30 it shows the dcmd use of ::fptrace within MDB. The output from that 
will tell us a lot. You could also enable extended logging in the 
/kernel/drv/qlc.conf and reboot. Then post back here and attach the 
/var/adm/messages along with the fptrace output.
