You might want to try Tab Clicking Options. I use a middle-click on a
link, or on a bookmark, to open in a new tab. A middle-click on a blank
area of the tab bar opens a new blank tab. Middle-click on a tab to
close it.

It says not available for SM 2.23 but you can mod the install.rdf file
in the.xpi package and it works fine.
Being a Mac user I take "extreme measures" just to invoke a right-click
(cntrl+click)...so middle click isn't really an option for me.
But it's been pointed out that I can use command-click ("splat"-click)
and open in an new Tab directly instead of using the contextual menu.
That's pretty nice option for use with my Magic Trackpad.
I really hate mice...I was a trackball devotee until I got this
Trackpad. After a bit of use I've grown to seriously prefer it.
