Wait. Do you want this for your helpdesk, your end-users, or both? Shouldn't address, phone, etc., be coming out of HR?

However, I don't see my end users adopting it, even with a howto guide. I think the tree on the left is probably too scary to navigate, and all those ou's and dn's and cn's would make their heads explode. Perhaps I'll have to make a custom form of some sort in php. That won't be pretty.
