The prophet is addressing the Covenant people of the God who changes not. 

The people of God, way back when, were identified as the Nation of Israel.   Physical circumcision was their "birthmark." 
 
jt: God's people in every generation are those with the right heart attitude. Paul writes in Romans (don't have concordance) that circumcision of the flesh is not where it's at, it has always been circumcision of the heart. The difference between their
covenant and ours is the power emanating from the cross in the person of the indwelling Holy Spirit. 
 
Today, Israel is given the same consideration as all those of the pagan world and circumcision is of the heart.  We call this contrast "change."
 
jt: God has not changed and neither have His standards. Pagans were always welcome when they met God's standards.
Think Rahab, Cornelius, etc. I know there are others but I don't have reference materials with me.
