Although it is some time since I looked at this, when I was looking at
it, I spent more than 5 mins on it. Yes, no replaygain, LMS volume was
100%, ALSA volume was 100%, no resampling, etc. etc.

100% sure that can be ruled out.

The "test" I use, is definitive enough for me. Not interested in
investigating further. Life is too short when there is other hardware
available which passes that "test". I've said before, I personally have
very little interest in audio out of HDMI port.
