I just looked at your document and the styles..  It is confusing I see the 
differences in the header styles but I don't see any difference between page 
styles..   if you look at your "left page" style it shows next style as "Left 
page" ???  However you have the margins the same between the pages..  ??  
Also left and right page styles do NOT have headers on.   It looks like you 
have a page stye called "body" ??, that you start with as default then the 
next page has a left page assigned to it.. from there on each page 
gets "left" 

When I apply the left page style there is no header turned on.    Are you 
confusing "page styles" with "paragraph styles"  I think you need some 
consistancy if you are going  to use different left and right pages.  

First off use "left and right Page styles" on the pages.  Then format the 
headers with what ever paragraph style you want to use..  if you want the 
same graphic on every page put it on the page in the background. 
<Format/page/background/image>

also you should not use returns (new paragraph) to move to the next page.. 
inset a break (page)..   hope this helps.. Fred
