All I can suggest is chapter 12 of the Writer guide ("Ch12 - Creating Tables
of Contents, Indexes, and Bibliographies"), available in pdf from this page:
http://documentation.openoffice.org/manuals/oooauthors2/index.html
Hopefully, someone will be able to tell you exactly what to do but, if not,
that may help.
