We are talking about the A4/Letter Avery label defaults:

File|New|Labels

For some reason my default is set to Avery A4, but as mentioned in my
other post, once set to a label run (i.e., you've created a new set of
labels) it then defaults to the last used. You'd have to use a new OOo
profile to see the A4 default & it doesn't matter what printer the
system is set for, the default is Avery A4.
I think you have misunderstood what the issue is about. If you select Avery A4 you get the whole list of labels for that format. If you then select one of these labels to work with and select Format -> Page you'll see that OO has set Page Format to "User" and size to 20.83 by 28.44 cm and this is not what is expected when you work with A4 size documents. Of course you can change it to the proper format and size but then when you later print the label sheet OO has set the printer to a page size of US letter; again incorrect to my understanding.

Ah, you're right, I did misunderstand. My apologies.

On my system (2.4.1 OOo - linux) I select a PDF printer w/A4 and then in
the File|New|Labels I select Avery A4/C9146 Photo Quality Label (Full
Page) and in the wizard it shows 21.00cm x 29.70cm (1x1). So I then
click the New Document button and the result is (from Format|Page):

Format: User
Width: 21.04cm
Height: 29.74cm
Left: 0.00cm
Right: 0.02cm
Top: 0.00cm
Bottom: 0.02cm

This (slightly larger size) does the same on a letter sized document and
I think that it is so that the template/frames will print to the edge of
the top & bottom to include all labels.

Can you select the same Avery A4/C9146 Photo Quality Label on your's and
see if you get the same/different?

Yes, I see the same. The wizard size, though, is the size of the label.

Can you try the following label: L7671? The wizard size is not interesting (I think). You should get from Format|Page:

Format: User
Width: 18.54cm
Height: 28.82

Which is far from A4. And apparently this "User" format is not the same with different labels. Weird.
