If we have little to no bleeding at the conclusion of the insertion, then we apply a biopatch.  We only change the dressing at 24hrs if it is bloody or no longer intact.  If there is active bleeding at the conclusion of the insertion then we will apply a gauze pressure dressing at the site cover with tegaderm and change it the next day. Occasionally, we have a kid who will require a gauze dressing under the tegaderm and a pressure dressing for a couple hours on top of the dressing secured with coban. We seldom have to do a 24 hr dressing change, and our kids appreciate that. They hate the dressing removal, and the chloraprep stings a lot when the insertion is new.  If we are able to wait a week, the kids are much more tolerant of the dressing procedure.

Was wondering if everyone is still doing 24 hour drsg. changes if you place a biopatch immediately after placement of PICC/ML.  Of course if there is no bleeding.

Are you placing pressure drsg on outside of sterile drsg? 2x2's and coban?
