A QR code is just a series of square dots arranged in a grid that contains information based on a standard. They contain information on whatever they're attached to. In my experience, they're frequently used so you don't have to type in a long URL or some other information. For example, I've seen QR codes on badges I've gotten attending a conference where scanning that code would take you to the agenda for the conference. You'll also see them on ads where scanning them will take you to the web site for the product or the company.

I like Digit-Eyes the best.
The key is to place the camera against the object, start the scanning, and 
slowly pull the phone
away about 4 to 8 inches. If you aren't on the bar code, you can slowly turn 
the item about a
quarter of a turn and wait, or, start over.
