I assume you somehow are missing a step. Are you changing the default voice or are you adding a second voice? If you are adding Tom as a second voice then after you have done so you need to turn the rotor to languages and flick up or down to select it. Otherwise maybe give Apple Accessibility a call and see if they can walk you through it step by step over the phone.

The tom voice is selected but it won't change. We were able to do it the other day but it was a bit difficult to do. once you choose another voice, and then have Samantha has the voice does it some how get more difficult to change voices?
