No, but I think that the approach you suggest would work out well. Create the
form once and forever after all you'd need to do is edit the questions.
Verifying who actually takes the quiz could be problematic but I seem to think
that email addresses can be automatically captured. Go for it... The school
administrators might have a problem though.
