If you're just going to use formmail to send you the answers, a standard
form will probably work and is fairly easy to set up. You would either have
radio buttons next to the answers (for multiple choice) or text boxes for
them to type in the answers. Each field would be given a unique name so
formmail will pick it up and include it in the email to you.

That's how I'd do it.
