Dick,

You haven't looked hard enough.

I have seen many.  Some of them written in javascript -- I recall 
long discussions on how you could hide the answers.

I have a quiz up that I wrote long ago:

and of course you can grab that script but it must be horrible.

and you might want to look here:

and look at the top 50 pages.  goodtimes.html will be there.  There 
are webpages that just contain links to quizzes and tests and 
goodtimes.html is on some of those. You might find some leads or at 
least some ideas.
