The Broadband ISPs charge a premium in high cost passed on to the consumer
in terms of not only access to use their network pipes but for equipment
upgrades for example the cost of a Baystack Ethernet Routing Switch or
Multiservice Switch's
which can cost between $1500 to $6500, cableing, servers, voip servers, ISP
monitoring software
ect which factor into some of the bulk cost.
You also have to factor pay in salaries for the network techs to wotk on those
upgrades, service and
support, advertising, marketing which another part of the cost goes in to
support.

They can make money, but i sometimes feel they do things to maximize the
cost. I.e: It's more advantageous to make people upgrade to a higher cost
plan to get better upload speeds (it was the big draw for me - and it's
costing $240 more per year). I expect it will remain so, even when there are
few technological reasons for a 1.5 meg down and 128 up. (When i got
my 1.5meg, it was 384 up.)
