Some notes on your comments:

This updater was rolled into the 12.1.9 updater. There are only two updaters
you need. (those with a newer DVD only need the 12.1.9)

Did you restart between each phase of the install? Did you quit all
applications before running the updaters?

Install from DVD...restart
Install SP1 ...restart
Install 12.1.9...restart

You are not fully updated.

"Remove Office" in Office 2008 does not remove everything. It does not
remove the following. You will need to manually remove them to get a clean
install.

/Library/Automator workflows

~/Library it does not remove any Microsoft .plist files or the Microsoft
folder in the Library folder containing files and prefs.

Simply drag back the Office for Mac folder in Applications from the previous
version. You can zip this folder after each update to get that version as a
backup.
