2 things 
1) I had a similar problem when I didn't compile all the source code. So if
you are overlaying a file system you have already compiled in make sure you
remove all the object files and rebuild everything.

2) Assuming that doesn't help I'll add some debugging to the Xv init code
and we can debug what is wrong.

I'll ship you a new version of ivtv_xv.c later today.
