Like at Old Dominion, we only use one date for the entire policy.  At UT 
Austin, we update the policy's effective date when a major revision occurs to 
the policy language.   Minor, editorial type changes do not trigger a date 
change.

Our standard policy format now has a section called "Responsibilities and 
Procedures".  As review of our legacy policies occur, we're attempt to keep 
procedural detail to high level description and point the reader to a 
divisional website or to forms to find out more detailed procedural steps.

Hi Eunice,

At Old Dominion University, we only have one update date for the entire policy, 
which has a section on Procedures.  However, the content of that section will 
sometimes contain only an introductory statement and a link to the procedures 
that are in separate document, especially if they are very complex and may 
change more frequently.  That way, we do not have to update the policy every 
time the procedures are changed.

Hello all.

For those of you who have a combined document - policy and procedure - do you 
allow separate update dates (edit, revision) for each of those sections 
(policy/procedure), or when a document is updated, do you change both dates?  
In the past, we've had folks update both dates, and I'm leaning towards going 
the other way.  So I figured I'd ask to see if there is a "usual" practice out 
there.
