I chose to do something similar, directing users to a specific page on
my server:

http://www.dailyroads.com/voyager/m/issues

In the app I have a menu item called Issues, where this link is
displayed. And there is also another link, inviting users to leave a
comment, if they experience any other problems.

This seems to work, as I hear about problems directly from the user,
and I can write back asking for specifics, or even sending a new
version of the app (if I could fix the problem in a short time).

Maybe this is one of the reasons my app never got below a 4* rating on
any given day :)
