I experienced this exact error the other day in this exact scenario. My Table was based on a View form that wasn't setup properly, the 'request id' field as defined wasn't unique, and when the browser tried to pull up more than one entry with the same request ID it gave that error. I ended up re-defining the db view to make request id unique and my problem went away :)
