On my system second paragraph doesn't highlighted when I remove all my
custom files and use only standard asciidoc highlight.
So, maybe we're using different versions, or you've some non-standard
fixes. File /usr/share/vim/vimfiles/syntax/asciidoc.vim on my system has
size 10666 bytes and md5 98882aaf67dd1c907671fe2f49417949. I'm using
asciidoc 8.6.7.
If you're using newer asciidoc, then maybe this issue was already fixed in
standard highlight after 8.6.7, but my conceal implementation contain some
copy&paste from 8.6.7's standard highlight (with some modifications), thus
it may overwrite newer fixed highlight with older version. But I don't
really think this is the case - because my conceal highlight doesn't touch
literal paragraphs at all, it change only inline things like bold/italic.
