You are right about TOSLINK. It is some cheap, plastic "fat fibre" multimode LED crap. Really horrible and asymmetrical rise/fall times.
The 1 km and above refers to single-mode laser systems. The lasers do not want to have all that light bouncing back at them from very close ranges. Lead to lots of heated debates in the lab 25 years ago. We had to characterise all the fibre (as it was new technology, and flaky in a sense), but we needed to have at least a few km of fibre to look at. Could it be still on the reel, and possibly have cladding light fool us, or did we need to string out all over the building first? We took the lazy way out, in case you are wondering. And be glad that you are not an engineer. Look at what it did to me!
