My standard location will be like this:

São Roque, Pico, Açores

I don't use abbreviations and I don't state Portugal after Açores. I don't use Azores.

For a city with more than one freguesia:

São Pedro, Angra, Terceira, Açores

For a location on the continent:

Mercês, Lisboa, Portugal

Of course, if I don't know the exact freguesia, it might be:

Angra, Terceira, Açores
Ponta Delgada, São Miguel, Açores

I don't pay attention to concelhos, unless that is how they are so far known.
Sometimes the birthplace is listed as "desta concelho" and if it's a record from the Angra concelho, I enter them as:

Concelho de Angra, Terceira, Açores

Once I find the exact freguesia, I change it over to the above format:

Doze Ribeiras, Terceira, Açores

For USA locations, I always include the county:

Oakland, Alameda County, California

I don't include United States of America. But if I know they only came from the USA, then I will list them as:
United Stated of America

Sometimes in the records when a person is living currently elsewhere then the baptism is taking place, they might be said to be living in America do Norte (United States) or America do Sul (South America).

For locations in Brasil, (I don't write Brazil, since they call it Brasil), I write:

Santa Ana, Rio de Janeiro, Brasil

But people from Brasil will abbreviate states like:
Rio de Janeiro - RJ - Brasil
Niterói - RJ - Brasil

What do most of you people do for this?  You are entering a birth, baptism, marriage, death.  It doesn't matter.  Your screen opens up and asks for the place where this event happened.  My program has the following fields:
Detail
City
County
State
Country

Detail = the church, but I use it only if there is more than 1 church in that freguesia (Sao Miguel Arcanjo & Sao Pedro are both in Vila Franca).
City = freguesia
County = I put the island there.  I know it's closer to the concelho, but before the CCA came online, the concelho didn't matter.  I pulled the roll of film for my freguesia.
State = I put Azores there
Country = Portugal

I wouldn't too much work to put the county as the concelho and put the state as Sao Miguel, Azores.  But in sharing Gedcoms, we probably should start to be consistent with each other.

And I DO NOT abbreviate any more.  Or not in the places (in my side notes I abbreviate).  SM?  What's that?  Santa Maria or Sao Miguel?  F?  Faial or Flores??  You want your great-grandkids to inherit this and as adults they pull it out and can't figure out your abbreviations? 

So how do all of you fill in your place fields?
