Definately not .jpg.jpg; I always have extentions visible. All filenames are folder.jpg so nothing odd there. Nothing common as to size, etc.
The only possibility I see of those named is garbage in the jpg file; I'll play with that and see what I come up with. Yes, I am using flac, but I would prefer not to embed them all as that isn't the way my current tools work and I'd have to manually touch 1,000 albums. It ought to just work...
