+1 I am dealing right now with the end result of what you're dealing with Dan... it sucks.
Do the right thing by the company in this case and record the incident. Management should take into account the "we're all human" aspects, but if this is an environment where simple things are getting missed, or done wrong, not reporting it will only hurt you.
