Sarah, I believe the most common physical symptom is internal bleeding.
You will see little blood spots under the skin, that are called petechia.
When I was fostering the Cavalier that had this, I was told to look at his
gums and stomach daily to check for these spots.  They are usually quite
small.

Something that I don't think anyone has mentioned is that, besides being
congenital, thrombocytopenia can be vaccine-induced.  This was the case
with the Cavalier that I fostered.  He had his vaccines, and then I believe
he was going to have his teeth cleaned, so the vet did pre-anesthetic blood
work and discovered his low platelets.  The thrombocytopenia was treated
with prednisone.  The prednisone caused him to develop diabetes.  The
uncontrolled diabetes caused him to go blind.  In spite of all his
problems, I still believe he may be the sweetest Cavalier I've ever met,
besides my own of course.  Very loving and trusting.
