If the dog is 2 years old or older then once is enough. You can do a pre-lim on a dog under two with better than 90% accuracy. I usually do our dogs at just over one year of age and get the patellas certified at the same time. A regular vet can do the x-rays, but not all vets are equally adept at positioning nor do they all have up-to-date x-ray machines. The vet work for x-rays costs me about $150 per dog so I go to a vet with the latest x-ray equipment and who is good at positioning. No sense wasting money and taking the anesthesia risks unless the vet and equipment are topnotch.
Sometimes I will have the dog done again after they reach two years old and sometimes I don't. It depends on the earlier pre-lim rating and what I thought of the x-ray. I look at every one of them myself. I would not bother to have a pet done unless it was limping or there was a major concern. Cavaliers can have moderate HD and never show it. That is one of the problems for breeders, that a dog can have moderate HD and show not symptoms. This makes it very difficult to find an outside stud when so many Cavaliers are not hip checked. You never know what you are bringing into your line. I prefer to buy a puppy, raise it and do the testing myself. That way I know what is what.
