I meet most of the families @ the time of admission, ususally just briefly, to schedule their family members careplan. At my current facility the attendance of families is very highl, usually 90-95% of residents and/ or families attending. This holds true for even for those residents that have been here a long time and are fairly stable. I alot them 30min increments for the careplans. I also meet with them to discuss any problems that arise, usually spur of the moment.

I fortunately do not need to meet with the families unless there are issues that the unit managers, social worker, or business office cannot handle.  When a patient is cut form Medicare it is no surprise to the family and they are well aware why and when.  The entire team is well versed on Medicare coverage from admissions to the charge nurses.  The charge nurses do not retain the information but they are aware.
