At our facility we had the following assessments: skin which we did a head to toe inspection twice a week and a risk assessment quarterly, falls which was done on admission and quarterly and the same with th elopement assessment.  Siderails we had no special assessment tool but were looking at one.  We found  that too many assessments are just as bad especially when they were not getting done in a timely manner.  We too were looking at what we were doing and trying to get rid of the redundancy.  The documentation got out of hand.    Denise F.
