My computer allows me to close it as a 1/1. My computer system will not
ever allow me to change the reason for assessment.

I may plan on doing an admission assessment, but when they leave I don't
do it. For example I usually schedule my 5 day with my admission. So, my
assessment is all coded all disciplines have signed it, but I still
haven't done the RAPS. If they leave at 10 AM on day 14 and I haven't
done the RAPS. I just write right on the RAP summary sheet. Resident d/c
prior to completion of RAPS and CP. Then I write dashes in the proceed
to care plan column. I have never had a problem with this, and have
always done it this way. I do the same thing if they are a significant
change and d/c or die before RAP completion date.

So what do you do when they don't leave on day 14, doesn't that
admission assessment need to be completed and signed by day 14? 
So if you do a 5 day on day 5 and code it 0/1, when do you set your 14
day assessment and complete it, in order to stay compliant with OBRA?

I also work in a hospital-based SNF, and do not do the OBRA Admission
Assessment if they leave on day 14.  I code my first assessment 0/1 and
do a d/c tracking 8.  Here are some other answers given to this question
recently:

The top of page 1-15 states "An RAI  must be completed for any resident
resident residing in the facility longer than 14 days, including:" So it
would not be "longer" than 14 days if the resident left before the end
of the 14th day.

If they left before 11.59 PM on day 14 , an admission assessment is not
required.  You need to keep any assessment that you started, but did not
complete, on the resident's medical record and make a notation about why
the assessment was not completed.

If any experts out there (Rena? Jan?) disagree, please respond.
