It is of utmost importance that we all provide resident focused and not
financially focused care. Having said this, it is also important that the
most advantageous RUG level be reported in order to pay the facility
accurately. CMS provides a range of allowable Merdicare ARDs. It is critical
for the Director of Rehab to meet daily with the RNAC and together determine
the appropriate ARD. Flexibility from both the therapy department and the
RNAC are of vital importance for survival in the "land of RUGs."

Subject: moving PPS ARDs



We have a new therapy company.  On their first day they asked me to unlock a
PPS MDS and change the ARD because they thought they could capture a higher
RUG score.  I refused.  Now they are asking me to change the ARDs soon after
I post the schedule.  This does not usually involve RV-RU groups-often RM
and RH.  I think this may be legal as they are asking me to change the dates
prior to the ARDs but I'm not sure this is ethical.  I definitely don't
think it is standard practice.  Can anyone validate this?
