No, you don´t need the kv file neither on fastdl nor on client it´s only
for the server. The only thing you have to do with custom maps is to put
files on server(bsp, kv and perhaps nav), fastdl (bsp.bz2 and perhaps
nav.bz2) and add the map in gamemodes_server.txt to a mapgroup. Where I
am not sure it is needed generally but for sourcemod voting system is
adding it to mapcycle.txt and maplist.txt.
_*Now here´s the solution for de_nuke_ve:*_
The engine seems to have a problem with two underscores in mapname. I
found this solution yesterday in a forum and tried it instantly because
I was almost getting crazy because nuke_ve was the only map that didn´t
download. Y*ou simply have to rename de_nuke_ve to de_nuke-ve and it
will download!* It would be great if someone from valve either could
include a fix to the problem into next update *OR* direct this issue
right to volcano so he can rework the map to de_nuke-ve, because if you
rename it you have some missing/white textures e.g. on truck at t-spawn
and on gas tanks.
I hope I could help everybody having issues with the map.

gamemodes_server or your server.cfg?
