Scott,

I seem to be having a simular problem. I did have it set to send two
different bounces, one to the sender and one to a mail box to help analyze
the spam being recived, but only the first one went. When SpamReview was
released, I thought I would try the HOLD action for the second test, nothing
has been held, and I send out hundreds of bounces a day.

I am running 1.34 and it was not working with erlier versions as well. (Just
started trying HOLD on 1.34)
I do have all the other tests set to WARN to provide more info. The TAG
tests were sent to the special mailbox.

Yes. We are currently investigating a problem reported with v1.35a that
may cause the WARN action not to be used when the HOLD action is used. If
you are running 1.35a, that could be why this is happening.
