What information about DNS updates appears in the dhcpd log file?
Successful dns updates will report success, failed ones will
report failure.  From our site's logs, here is an example
of an update that failed, because the name was already present:

Mar 25 10:34:56 lax dhcpd: DHCPREQUEST for xxx.xxx.xx.xxx from 00:10:4b:9e:82:4b via 
ef0
Mar 25 10:34:56 lax dhcpd: add failed: hostname.domain. IN A xxx.xxx.xx.xxx
Mar 25 10:34:56 lax dhcpd: DHCPACK on xxx.xxx.xx.xxx to 00:10:4b:9e:82:4b via ef0

(I edited the fully qualified domain name and IP address assignments
for privacy.)

There is a corresponding entry in the dns log for the failed transaction:

Mar 25 10:34:56 lax named[83]: error processing update packet id 41558 from 
    [192.52.153.11].3284

If you are not seeing entries between DHCPREQUEST and DHCPACK for dns updates,
the installation of the patches apparently did not succeed.  Check the source
file server/dhcp.c to be sure the calls to nsupdate() were properly applied.

There are two changes made by the patches, one for releases and one for adds:

    /* If we found a lease, release it. */
    if (lease) {
        nsupdate (lease, DELETE);
        release_lease (lease);
    }

and...

        lease -> timestamp = cur_time;
        if (!(lease -> flags & STATIC_LEASE))
            nsupdate (lease, ADD);
        dhcp_reply (lease);
