In the step 4, in what size is the empty sheet?
File - Properties menu of Adobe Reader would show you it in the tab 
"Description."
Does the width of the sheet equal to the height of that?
Is the size of sheet in the PDF file same as that of page size in the Writer?

In the step 5, which do you mean?
 a) Once the OpenOffice.org has started behaving problematically, the symptom 
occurs all the time with any documents.
 b) Even though the symptom can be observed several times over the years, the 
problem sometimes does not happen; sometimes happens.

Could you tell us a precious version number of the OpenOffice.org or its 
variants?
To identify it,
 1. Help - About OpenOffice.org
 2. Sequentially press S D T keys with holding a Ctrl key in the dialog "About 
OpenOffice.org"
 3. Memorize a text like "[310m11(Build:9399)]" which will be shown in the step 
2.
 4. Press a ESC key to close the dialog.

The text seems to come from one of the configuration files in the source tree.
