It turns out that it wasn't as simple as I'd thought and a bit more 
experimentation reveals some new clues:
1. Everything works as normal in the Xfce desktop.
2. In KDE, Youtube, games, etc work fine *until* I start Amarok.  Then some 
sound stops working as before.  (Also Einstein has to be killed multiple times 
before I can get rid of it.)
3. Closing Amarok doesn't help.
4. If I log out and then back into KDE, the situation is as per 2. again.
5. I've uninstalled and then re-installed Amarok, but apart from throwing away 
all my covers, etc, sound is still broken.

So I can listen to sound on websites and games OR I can listen to music with 
Amarok, but not both, unless I use Xfce.  Anything else that might cause this?

I must have misread the message.  I get a different login screen now, but 
presumably I can change it back somewhere.
