Ok that clarifies things for me.  GPLv2 is the only valid version.

The text you quoted is the FSF's recommendation in COPYING on how to
apply it.  That recomendation was not followed.  Thefore a later version
of the GPL is simply not an option.  As no one agreed to any later
version.

Absolutely.  Anything else would require the signoff from all historical
contributors to be valid.
