Reading the MSN web site is pretty confusing to me, as it describes several subscription offerings. More service, or another subscription isn't needed. I have discovered that creating a new account in Entourage offers the option of "Hotmail/MSN" as an account type. Creating an account with that option, by entering account-id and email address, creates an Entourage Hotmail account. It appears to be pretty much like an IMAP account.
