All of the above is true, as far as I know. However as long as you are 
careful not to do anything to reset the counter, you can use the file name 
to determine the count.

Nobody has mentioned that the folder number, such as 154CANON, indicates the 
number of hundreds of photos, with the counter starting at 100 rather than 
000. So if you subtract 100 from that number it gives you the number of 
hundreds of photos. E.g. 5400 in the case of 154CANON. I don't know how high 
the folder number will go before rolling over.
