Log message:
three changes here:

1) Fading code now allows a real handle to be specified again.  :)
2) Restore insturments when we switch songs.  (like, say, when you gain
points and the previous song is interrupted briefly) -- test please!
3) Now sends regular midi CC events again (volume, panpot, etc) again.
It was previously squashing all CC events.. I left out the actual
midi_event call.  :)
