Log message:
1)  Fixed a dangling else problem in the Fade code.  Fading 0x0000 now
works again.
2)  Now initializes the MT-32's rhythm map, partial reserve, and reverb
stuff if such things aren't in patch.001 (or the patch isn't tehre
at all)
3)  Soundserver_null no longer terminates if the needed patch.xxx file
can't be found.  (another screwed up if statement)

This should fix #150, but until I get to my MT-32 on Monday, I can't
verify this.

KQ4 should now run again.  :)
