I had this problem at one time and had to work off my desktop, but as
crazy as is sounds, we tested naming the template with lower-case
letters and it worked! Please try this.

I seem to have fixed my problem of the vanishing static background
image. 
Turns out I have to be working off my desktop.  We have a Snap drive
where
we archive all our files.  When working with files on the Snap drive
there
seems to be a communication error somewhere and when the file is imposed
the supporting files cannot be found.  When I work off my desktop
everything works perfectly.

This is prepress 101 basics, but I guess we all need a refresher every
now
and then.
