If you only have the problem when you try to open the Extensions Manager, and not while cold starting or restarting the computer, it might be a memory allocation issue. The Extensions Manager, and a number of other Control Panels are actually programs. Try allocating more memory to the Extensions Manager. (Done from inside the "Get Info" box if you don't remember where to do that) Then restart the computer and try opening the Extensions Manager.
I discovered this strange problem back when I upgraded from OS 8.6 to OS 9 on a machine that was absolutely stuffed with Extensions. If allocating more memory fixed the problem, you might want to do a little Extensions housekeeping, and move any unnecessary ones to the Disabled Extensions folder.
