Thanks to everyone who answered my question about providing my Social Security Number to the people who run this contest.
I had written to them earlier with my concerns about identity theft and just received their response. They said that they normally do ask for the winner's SSN for tax purposes. However, they pointed out that since I was concerned about giving out my SSN, They said I could disregard the line asking for the number. Sorry about the off-topic question.
