I seem to remember several Wizards and Warriors games where you saw skulls as bosses. They were of course huge and required quite a few hits to bring down. You' either have to have four swords and then a constant supply of them in the area, or a single sword with unlimited uses, which might be problematic from a programmer's perspective.
