I had this also. again it's random. once it crashed when I was changing
weapons, the other time it crashed when I was running through the passage
and another time it crashed after it gave me the game over message.

Yeah, it has crashed a few times on me without changing windows as well.
One time I pressed the number 4 to select the Uzi and the next thing I
know I was on my desktop wondering where in Hades my game went to.
