That's absolutely right Horst, you can have mysql or PostgreSQL performing data replication with a few seconds/minutes delay on a server in the room next door all automatic and happening near real time (or real time if required).
All bells and whistles such as relational integrity, throughput and other enterprise level functions fully available to the practice. Like everything, the practice needs to know what they are doing.
