This might be my final post to this thread (unless I'm asked to
respond some more).  I ran some tests on Tiger (10.4.11) to see what
happened with the two check-box settings in System Preferences ->
Growl, specifically, "Start Growl at login", and "Show Growl icon in
menu bar".  With both checked, and Growl running, I did the following:

1.  Clicked "Stop Growl" and Restarted.  The Growl icon reappeared on
the menu bar, and showed that Growl was stopped.  Yet "Start Growl at
login" was still checked in the Growl Preference Pane, and the
GrowlHelperApp was still in my Accounts -> Login Items.  This result
was NOT intuitive.  What does "Start Growl at login" mean?  I clicked
the "Start Growl" choice in the menu bar's pull-down menu.  This
returned me to the original state.

2.  I unchecked "Start Growl at login" and Restarted.  Essentially,
the same as #1 occurred, but GrowlHelperApp was missing from my Login
Items.  This was more intuitive since I had specifically turned off
GrowlHelperApp, which prevented Growl from starting.  I rechecked
"Start Growl at login", and GrowlHelperApp reappeared in my Login
Items.  I also made sure Grwol was running.  So this returned me to
the original state.

3.  I unchecked "Show Growl icon in menu bar", and the icon
disappeared.  But "GrowlMenu.app" remained in my Login Items.  I
Restarted.  The Growl icon reappeared, and the check box in Growl
(Preference Pane) was checked again.  This happened by itself.  From
what I can tell, checking or unchecking this box only has a local
(current session) effect.

All of this is simply meant as documentation for what I'm seeing.
Hopefully the Growl team can make use of this information.
