Assuming your client Steam and srcds are both fully updated then the issue is some difference in your server.dll/.so on the server, and the server.dll and/or client.dll on the client. Unfortunately the HL2 core gives you no way (that I know of) to determine exactly what you have desynchronized. Compiler/linker differences are a possible culprit if you're certain the code base is identical.
